SportsNetInc. provides secure online registration services for
University Preparatory Academy Athletics.
As a means to streamline the UPA athletics registration process, the UPA Board of Directors has approved the completion of the following two forms prior to participation:
• SportsNetInc. Physical Exam Form
• SportsNetInc. History Form
It is recommended that all UPA families complete the required SportsNetInc. Physical Exam Forms during the summer, prior to the first day of school, to ensure there is no lapse in athletic eligibility during the school year due to expired evaluation forms.
Below are instructions for creating a parent/guardian user account and registering one or more athletes for the 2019 / 2020 University Preparatory Academy Athletics Program.

1 | Have your child’s correct UPA student identification number. |
2 | Have the exact spelling of your child’s first and last name as it appears in official UPA school records. |
3 | Have your child’s active health insurance coverage information, including the name of provider and policy ID number. |
4 | Have scanned copies of the two required SportsNetInc. Physical Exam and History Forms on your computer. |
NOTE | To complete the registration process, both SportsNetInc. Physical Exam and History Forms: • Must be 100% completed • Must show a date of completion after June 1st, 2018 • Must be uploaded to the SportsNetInc. Website |

6 | Click the Create an Account link near the bottom of the screen. |
7 | Enter the required information and submit the form. |
8 | Log in to the system with your email address and password. |
9 | Follow the instructions inside the system. |
10 | Once you have completed the entire process, ending with your final electronic signature on the last available screen, the UPA athletic administrator will receive notification of your online submission. |
11 | Once cleared through the SportsNetInc. system, a clearance e-mail will be sent to the parent/guardian associated with the account. |

NOTE | UPA student athletes will not be allowed to participate in any of the following activities until the entire registration process is completed using the SportsNetInc. Website. • Pre-Season Conditioning Sessions • Team Try-Out Sessions • Regular Season Practices or Games |
